INTERNATIONAL TRAVEL REQUEST
If you are playing outside of the 50 United States and the District of Columbia.
Please contact Val O'Keeffe with questions. Forms should be submitted to NCYSA 45 days before you travel (so we may process the paperwork and send to the Federation by their 30-day deadline).
GUIDELINES FOR INTERNATIONAL PERMISSION TO TRAVEL
US Soccer requires foreign travel papers to be submitted no later than thirty (30) days before traveling.
Items Required: Please DO NOT STAPLE the required items together
Two (2) Application to Travel Forms:
- A US Soccer Federation, Inc Application for Foreign Travel form (linked below)
- A US Youth Soccer Application to Travel form (linked below)
The Youth Application to Travel cannot be completed until International Approval has been granted from USSF!
Two (2) Certified Rosters with the NCYSA registrar's red stamp:
- one roster for the US Soccer Federation form
- one roster for the US Youth Soccer form
Ted Stevens Olympic and Amateur Sports Act:
One (1) Ted Stevens Olympic and Amateur Sports Act compliance statement signed by a team official.
Official Invitation or brochure:
Two (2) copies of the Official brochure, pamphlet, invitation or other applicable material of the Tournament or Games host.

Preferred is a letter of invitation on the club's letterhead
Appropriate fees- Two (2) separate checks, one (1) online payment
If paperwork submitted the week of travel, a late fee will be assessed.
